“My breast implants are too big and heavy and I have to wear a bra all the time!” This patient in her late 40’s had a previous breast enhancement with 650cc moderate profile subpectoral implants. Her breasts were pendulous and long. She wanted a revision where her breasts would be positioned higher, rounder and look like they were implanted and not big and saggy. Dr Pautler’s revision included a smaller gel implant (535 cc High profile Sientra), an extensive capsule repair (capsulorrhaphy) inferiorly, reinforcement with an internal Galaflex bra, and a full mastopexy. Her post-op photo shows a full, round breast, positioned so that daily bra wear is not needed.
